    Earlier this week was the Coalition to Improve Diagnosis in Medicine meeting, for which ASCLS is a member of.  The National Quality Foundation (NQF) and the Agency for Healthcare Research and Quality (AHRQ), who are both coalition members, have been working on a new Diagnostic Error reporting form. This form can be used by not only clinicians and other healthcare providers, but also patients and their families/caregivers to report Diagnostic Error. The goal is to collect sufficient data to identify specific target areas for wide-scale intervention. But right now they need our feedback on the data collection form.

    They have asked our membership, as many who are willing, to look at the form and submit comments. Looking for everything from technical fixes to improving wording, from items that are just plain confusing to information that it missing. They need our help to ensure this form is usable for both healthcare provider and consumer.
