Accelerate Progress Toward Population Health, Well-being, and Equity
Beginning in April 2020, the Institute for Healthcare Improvement will be convening an Action Community focused on improving population health. Building on the success of Pathways to Population Health (http://www.ihi.org/Topics/Population-Health/Pages/Pathways-to-Population-Health.aspx) and the framework and related tools, the Action Community will bring together a cohort of health care delivery organizations in pursuit of population health, well-being, and equity.
