Dear Professional Leaders,

The Patient Safety Committee through its Benchmarking Toolkit Workgroup is creating an exciting new Patient Safety product:

A section in this product will include 'tool templates' (forms, spreadsheets, example policy/procedure, etc.) to assist laboratorians in performing Laboratory Medicine focused Patient Safety quality improvement.

Critical Support Request:
Our committee is in need of example templates (spreadsheets, word documents, etc.) that facilities are using for Quality Improvement (QI) activities. Templates may range from basic to advanced as tools may be used in various laboratory settings. Templates may be either laboratory specific or can include templates used by facility QI/Risk Management departments.

Example Templates Needed:
-forms and spreadsheets to utilize in collecting and analyzing data
-forms and spreadsheets to define measures/indicators/processes/
-forms and spreadsheets used for standardized QI reporting
-spreadsheets that include graphs for visually displaying/tracking data
-policy/procedure that define Laboratory Patient Safety initiatives, selection of indicators, QI activities, etc.
**Confidentiality Note: All 'facility identifiers' will be removed prior to use in this product.

Deadline to Submit: Thursday, Feb. 20
